New Patients
Starting physical therapy shouldn’t feel complicated.
Here’s everything you need to know before your first visit, from scheduling your evaluation to completing your paperwork and preparing for your appointment.
How to Become a New Patient
Becoming a new patient at Amherst Physical Therapy takes three steps. Call our office at (603) 672-5125 to schedule your initial evaluation. You'll then receive an email invitation to our secure patient portal, where you complete your health history and intake paperwork before you arrive. Finally, come to your evaluation appointment at our Amherst clinic with your insurance card and photo ID.

01
Schedule your evaluation
Call (603) 672-5125. Our front desk will check your insurance benefits, answer any questions about coverage or self-pay rates, and find an appointment that fits your schedule. We're open Monday through Thursday, 7:00 a.m. to 7:00 p.m., and Friday, 7:00 a.m. to 5:00 p.m.
02
Complete your registration online
We've gone paperless. Once your evaluation is scheduled, you'll receive a patient portal invitation by email and can complete your health information from your phone or computer at your convenience.
03
Come in for your first visit
Your initial evaluation takes approximately one hour. Your physical therapist will review the history of your injury, assess your movement and strength, and discuss your recovery goals. Depending on your injury and pain level, treatment may begin during that same visit.

What to Bring
Your insurance card
A photo ID
Any physician referral, imaging results, or surgical notes you have
Comfortable, loose clothing that allows access to the area causing pain for knee pain, for example, bring shorts or pants that pull up over the knee
Completing your portal registration ahead of time means less paperwork at the front desk and more time with your therapist.

HAVE QUESTIONS BEFORE YOUR VISIT?
We know starting physical therapy can come with a lot of questions. From what to wear and how long appointments last to insurance, referrals, and what to expect from your care, we’ve covered the things new patients ask us most.
