New Patient Registration
Amherst Physical Therapy uses a secure, paperless patient portal for new patient registration. Complete your health information from home, on your own time, before your first appointment.
How New Patient Registration Works
New patient registration at Amherst Physical Therapy is completed online through our patient portal rather than on paper in the waiting room. After you schedule your evaluation, you'll receive an email invitation to the portal, where you enter your health history and intake information from any device. The portal is mobile-friendly, and once you're a registered patient our system securely stores your information for future visits.

Your Patient Portal Invitation
Your invitation arrives by email once your evaluation is scheduled. It comes from our partner, Pinnacle Rehabilitation Network, and our EMR vendor, with the subject line "Pinnacle Rehabilitation Network Patient Portal Invitation."
If you don't see it within a day of scheduling, check your spam or promotions folder. Still nothing? Call us at (603) 672-5125 and we'll resend it.
Why We Went Paperless
Completing registration online protects your health information and saves you time. Because your information is stored securely in the portal, you won't be asked to fill out the same forms every time you return to our practice. It also means less time at the front desk and more time with your physical therapist on the day of your appointment.

Your Privacy
Our Notice of Privacy Practices describes how your health information may be used and disclosed and outlines your rights as a patient. We encourage every new patient to review it.
After Your First Visit
Because we value your feedback, we use the Net Promoter Score, a nationally recognized method of measuring patient satisfaction. After your initial visit, and at points throughout your care, you'll receive a short satisfaction survey. We aim for a 10 out of 10 with every patient and use what we learn to improve care and recognize our staff.
